1. Registration Form - (white) completed front and back- all blanks filled or marked N/A.

2. Student Info Card / Media Permission Slip- (green) card

3. Physician Statement - (pink) signed by your child’s physician (the one on your old file is not sufficient; you need a new one)

4. Current Shot Records - with physician signature (the one on file for the current year is not sufficient; you need to provide a new copy)

5. Policy Handbook Acceptance - (purple) form

6. Non-Refundable Fees Acceptance - (yellow)

7. All Required Fees- the Deposit fee equal to one month’s tuition. Registration Payment must be made via CASH, CHECK or MONEY ORDER only

8. T-shirt Order Form - MCP will provide a t-shirt (for Pre-K and Kindergarten only).

9. Tuition Express form- only for families that are not already signed-up. It is required for all families to have a Tuition Express auto-draft account.
